• This free consultation offer is available exclusively to eligible recipients of the (National Disability Insurance Scheme (NDIS) and Home Care Package (HCP).
• Eligibility for the free consultation will be determined through an initial assessment via phone call.
• Once deemed eligible, consultations must be booked in advance and are subject to the availability of the pool and Exercise Physiologist.
• Appointments may be rescheduled or cancelled with at least 24 hours’ notice. Failure to provide sufficient notice may result in the forfeiture of the free consultation offer.
• The Clinic reserves the right to refuse the free consultation service to any individual for any reason deemed appropriate by the Clinic, without the obligation to provide justification. Such refusal may occur at any stage prior to or during the appointment and will not entitle the individual to any claim or compensation.
